• Fill in form below for each member of your family requesting to be added to NP Trudy's panel. Please read all fields carefully.

• Once this form has been submitted and processed by our admin staff, you will receive an email invitation to set up an Ava Connect account.  PLEASE NOTE: this invitation will only be active for 36 hours.

• Once you are signed up with Ava Connect you will have 3 months to book your Meet and Greet Appointment from account activation date.
